Medacs Healthcare are looking for a band 6 community nurse to work as part of the Dementia Team.
- Band 6 – Community Nurse
- Hours: Mon – Fri, 09:00 – 17:00 full time
- Experience within Dementia Care
- Particularly assessment and diagnosis work in Dementia
- Start as soon as possible for 12 weeks.
- Location: Severnfields Health Village or Hall Court
Summary of role
We are looking for someone to specifically work into the Assessment and Diagnosis pathway within the service. The role will be to support the management of referrals coming into the team alongside the operational lead and Nurse Practitioners in the service. The person will be required to review the referral information, and then review if any further information is needing to be gathered to enable the person to move to assessment. This will involve linking with primary care, wider services, as well as the patient and their family where needed. Experience in working within a dementia service and/or older adult setting is preferred. The role will allow for someone to have a blended approach in relation to mainly working from home but some time spent in the office will be required, particularly at the beginning when undertaking training/induction.
